Belt sander won't run straight and tight

My belt sander attachment has never run straight and stayed tight enough, even under light pressure. What am I doing wrong? I've tried every adjustment that I can think of and still the belt runs to either side and loses tension very quickly. All of the adjustment items are still in good condition....

Thanks for the information, but after reading your reply, I realize that the belt that I meant is the Gilmer (?) belt at the headstock end of the quill. At that location the belt is off of the grooved pulley by one or two ribs. I've worked on the lower V belt a few times and it seems fine, but the G...
